All forms required by the National Headquarters (including those in the Chapter Operations Handbooks) and many other publications are available in PDF (Portable Document Format) form. In order to view and print PDFs, you must have Adobe Acrobat Reader installed on your computer. Acrobat Reader is available free from Adobe's web site.
Many of the commonly-used forms are also available in Microsoft Word format as on-screen fill-in forms which can be opened and completed on your computer and printed for mailing to the National Headquarters. Simply download the form, open it on your computer, and type the required information in the gray-shaded fields. Use the TAB key to advance to the next field. If the form requires calculations of fees due, the form will automatically calculate the correct amount for you!
All items may be saved on your computer for future use. To do this, right-click (Macintosh users: control-click) on the item's link, then select the "Download Link to Disk" option (Internet Explorer) or the "Save Link Target As..." option.

Chapter Documents & Forms
The Chapter Personnel Update is used to report any new transfer students to your chapter or to report a change in memebrship status (such as active to conditional) in the middle of the year following the completion of the Chapter Personnel Report sent to you in the Fall.
The Fall Activity Report (due December 1st) and the Chapter Summary Report (due June 1) are both a means to report on your chapter's activities throughout the year. It is best to be working on these well in advance of their due date. You should be as thorough as possible when completing these forms and make sure to have all required signatures when sending it in.
The Initiate Registration Form is used whenever you put through a membership education class as members of your chapter. This form can be filled out BEFORE their initiation, but must be sent in no later than 30 days after their initation, or else a late fee will be added to each new initates' dues.
The Honorary Membership Form is used when initiating Honorary Members into your chapter. Much like the Initiate Registration Form, this form must be turned in within 30 days after the Honorary Initiation, or else a late fee will be added on top of the Honorary Initiation Fee specified on the form.
The Graduating Senior Report should be filled out at the end of each academic year. It is used in reporting your members who ARE GRADUATING that spring semester and is used in determining who to send to automatic alumni status on the next year's personnel report sent to each chapter.
These Colony documents are the most used in order to both apply to be a colony as well as to learn more about the process of becoming a chapter during the Colonization process.
Awards & Scholarships
The Awards Application is the most frequently used awards application for Kappa Kappa Psi. This form is used in the nomination process of most of our major awards, and depending upon which award you are requesting can require several other supplemental documents. Be sure to send in all required documents or else the application might not be considered.
Alumni Documents & Forms
The Life Membership Application is used when a senior or alumni member of either organization wishes to become a life member. Once you pay the (tax deductible) fee and submit your form, you should receive your life member card and shingle and be allowed to participate in all alumni activities and district/national conventions in an official capacity.

The Academic Achievement Awards Application allows chapters to acknowledge their members who have had successful semesters and high GPAs. This form is an easy-to-do application that will reward those members, just make sure to include some form of "Proof-of-GPA" so that the awards can be given.
